This essay shouldn't be about whether you made mistakes or whether your teammates made mistakes, but rather about what you learned about teamwork and interacting in teams from the experience. This is really a "what did you learn" question. And the more you can explain that and express how you applied what you learned to how you perform today the better. Don't get too caught up in explaining the details of the experience.
