I'll go with (5)

(4) has an ambiguous reference to "it": "Reacting dramtically to the traditional tailored look in women's wear, designer Emilio Pucci helped to create the "mod" fashion craze at the height of its popularity in the 1960s."

Does the "its" refer to "mod fashion craze" or "traditional tailored look"?

I pick 5 (E).....Only E conveys the meaning correctly with no ambiguity, even tho it is wordy.

5 is the best.

4 - logical meaning of sentence is wrong
3 - the middle phrase does not modify the opening phrase accurately
2 - 'his' is the wrong pronoun
1 - wrong modifier for opening phrase
