Can someone help me???

I took the GMAT 5 years ago right out of school. Never studied; 540. Not a very good score. I have heard that if 5 years lapses the programs you apply to will not see your first attempt. Can anyone tell me how to find out my exact test date, which i forgot, so that i am sure to gap it and basically erase the first attempt?
