You need at least a 720 with your GPA, and ideally a 740. Retake the GMAT.

As for your schools, given your healthcare career goals, swap out Kellogg and Booth for Wharton and Duke (as the latter two have the strongest reputation for healthcare management of any b-schools).

Stretch: Wharton, Sloan, Haas
Sweet spot: Duke, UCLA
Safety: USC

You have your six schools right there to apply to (and that is assuming you can score a 720+ on your GMAT).

HBS and Stanford are likely long shots. If you want to apply go for it, but just know that they're likely lottery tickets for you.
