Also, as a future career changer, I'd love to apply for a pre-MBA internship. Does CBS help w/ this effort? At what point is it appropriate to begin applying as an incoming student of CBS? At the top of my resume, I was going to write something like:
Graduate Education
Columbia Business School, New York, NY
Full-Time MBA
Incoming Class of 2019

For 1) this is definitely something you should tell the admissions office. I think they are pretty accommodating for this kind of situation.
For 2) just my personal opinion that you're over thinking about this. a lot of companies have multiple offices and it's nothing out of the ordinary that you would put the office headquarter address.
For pre MBA internship, I haven't heard of CBS helping in any official capacity, since you're not a official student yet, just an incoming one. It would have to be self initiated. You can start identifying yourself as an incoming CBS student now if you want to. Also I think the convention is to put "MBA Candidate" instead of just "Full-Time MBA" on your resume. Not sure where the convention comes from, just from what I see on social media and resumes.
