Great questions.
I do think that your low GPA and incomplete coursework will count against you, but I don't think it will make or break your application if you can clearly explain them and take responsibility. For EMBAs, I think they are much more concerned with your GMAT and your work experience and much less concerned with academics as it was such a long time ago. Still, the fact that you had a low GPA in undergrad and then again in your Masters program may make it seem like you can't handle academic environments. Make sure you quell that concern via an optional statement.
I think your startup experience is going to carry you pretty far, as is your consulting and industry experience. I think you've got a good list here- I might suggest adding one 'safety' type school like USC or something like that as your GPA/incomplete coursework could be a wildcard component.
They do like to see company sponsored applicants, I believe.
