Thanks for sharing your profile! Here are my thoughts:
Your academic profile is alright - though your GPA is slightly low, I don't think it's enough to disqualify you for top programs, especially in light of your stellar GMAT score and CPA and FRM exam success.
Your professional experience is somewhat unique. Not that many people come from a sales background, so the applicant pool may be slightly less competitive. However, it really depends on the depth and breadth of your accomplishments and on-the-job leadership experiences. I'm assuming these are strong!
Sounds like you are lacking the extracurriculars and community department.
All this said, your biggest barrier is going to be the fact that you already have an MBA. You'll a) have to check if the schools you are interested in will consider candidates who already have MBAs (some don't!), and b) have a very strong career and why MBA story.
As for your questions:
1. Like I said, I don't think your GPA is going to be the barrier here. I think that your lack of ECs/community activities along with the fact that you already hold an MBA is what is going to be a disadvantage. As far as some top schools you might consider - I'd check out Chicago, Cornell, NYU, Yale. I'd also include 3-4 slightly lower tier target programs.
2. With the above in mind, I think H/S/W will be stretches. Outside of your GMAT score, I don't see anything in your profile that really fits with the candidate profile for these programs.
3. See above - it's possible, but not that likely given the lack of ECs, community, and current MBA. However, your GMAT score could be compelling to some programs, and if you can weave a strong story around your professional experience and maybe bring in some personal stories to balance it, it's possible. I'd target schools below this range, but include one or two as reaches.
4. See above.
5. See above.
6. Safe range would likely be in the 30-40 range of programs.
