I appreciate your candor and honesty about various aspects of your profile such as your GPA and the story of your work experience. Keeping an element of that honesty and including some genuine explanations in your applications and potential interview can help a bit with the admissions committee. But, there are still a lot of potential weaknesses and concerns within your profile that maybe difficult to overcome and may make your chances at a top 15 school difficult.
My sense would be that the risk of sounding like you've done a lot of job-hopping and/or are uncertain about your career path is the biggest weakness, even more so than the GPA. While you've had some worthwhile achievements in that experience and gained entrepreneurial experience, the various experiences combined with the fact that your career history is shorter-than-average brings up that concern. You may have a better chance if you stick with your current scenario another year to gain more credibility/longevity with it and continue to add experiences and accomplishments. In terms of this year, you'll want to really work on nailing down your story of how your past leads to the MBA which leads to your future goals, and look most closely at schools in the 15-30 or so ranking (US News).
