Quite the contrary! Actually I need MORE info from you. Especially about volunteer work, extra currics and stuff like that.
But let's work with what we have.
Sooo first things first: Your job sounds awesome. And sounds like YOU are doing exceptionally well. Here too I need more detail to get a clearer picture, but basically very good. Now lets assume you can couple that with a great GMAT score (720 and above), you should be able to apply to Top 10 schools. (although depending on the detail of all the above you may want to open your list to Top 15 or 20).
Okay, so you did poorly on your Masters. Doesn't look great, but if you have excellent undergrad grades plus an excellent GMAT plus a great job, it's something that can be forgiven.
To answer your specific questions:
1. I think one thing has little to do with another. You apply to a full-time MBA if it makes sense for you to take those 2 years to do a full-time MBA because it is that which will help you reach your goals.
2. and 3. If you are doing so well in your job, I can't see WHY you want to switch. Basically, just keep on moving up like you have so far and you'll be fine. Make sure you have enough extra though - volunteer work, other activities, esp. leadership...
