Let me first start off by saying that due to your age, I would encourage you to only look at one year programs. If you graduate from b-school at 36 or 37 years old, you might face a difficult time finding an entry level job. Also, given your age, I would also encourage you to consider an executive MBA program -- as you will have a peer group whose experience matches your own.
However, if you are set on returning to school full-time, I would recommend focusing on those programs that have a history of accepting older students -- like IMD, Rotterdam. Judge and IE. If possible, attend an in-person admissions fair and speak to admissions representatives and see how they react to a candidate of your age.
Lastly, you might consider taking the GMAT again as your score is lower than that of the peer group by which you will be compared.
