Please provide me a honest and realistic opinion of my acceptance at my dream and target schools. Also please provide suggestion on other schools that will be a fit to my profile & post MBA goals

You will most likely not get into Stanford full time program at your age and experience level. If I were you, I'd stick with Sloan one year program or Wharton MBA for Executives, which will value your leadership and international achievements more than the FT program will. Carnegie Melon would probably work for FT program. Make sure you articulate what has held you back from your goals and why it's taken you so long to figure out you needed the MBA.
