Well first of all, you have very good numbers. So that is the basis you definitely need to start with to have a shot at the schools you listed, and you should have a shot. A lot of course will depend on how the B-school perceives your work experience, and for the moment this remains unclear to me as well (exactly what you did, whether you have leadership experience, how big your role was, how successful you were, etc.).
Job-hopping in of itself is not too much of a problem, as I feel it is something that can be often explained away. The most important thing (more important than the excuses and reasons you have) is to try and show each and every job change as an IMPROVEMENT or a sort of promotion. When you move companies, we want to see higher responsibilities, more salary, more leads, etc.... and if you can write such a story, than moving around becomes an advantage as opposed to a disadvantage.
I would say that Kellogg/Fuqua are likely stretches, (but you should apply anyhow) and that the other schools on your list are about right for you (although you could add one or two schools which would be "Safe schools"...McCombs is maybe a safe school for you)
