Because author presents an arguememt - More women sufer with Alz. than men because they have longer life, the NEGATES it. Says that is not correct, then gives reason what else could be causing it/preventing it. i.e Estogen.
Only A properly expresses an alternate to an existing theory, thus correct

For the main conclusion, the correct answer choice will be rephasing of the main conclusion of the argument.
i.e:A decrease in estrogen, rather than longer life span, may explain the higher occurrence of Alzheimer’s disease in women relative to men.

I agree with A.
