Recommendor's Recommendation format? [#permalink]

Have people got across the recommendation questions and format for the Fall 2008 applications for different schools? Can someone share them if he/she has them?

You're probably better off going to the school's websites and downloading the PDF. Most of the schools (if not all) accept and prefer that the recs be submitted using their online form, but they make the PDF available.
Most of them have a variety of multiple-choice questions - ranking the applicant on maturity, communication skills, creativity, etc. And then there is some form of short essay or open letter.

I was able to get the last year's format for Chicago GSB (got it from a friend). Can you please help me with the links for the formats for other schools (if you have them) ?
