"Each year" in the opening of sentence makes use of "annually" redundant in option A B and C ...
between D and E ..
You save something by X ...rather than you save something in X ..
Thus E is best choice .

E is good. through improving performance of workers is not good with D.

I think E is good, by preventing shows the method how companies could save money and also they refers to workers which is at right place....any comments?

This seems to be a parallelism question i.e.
companies could save by preventing illness (verb noun) and gain through improving performance (verb noun)....
A is my ans

....companies could save if they prevented illness (verb noun) and gain through performance improved (noun verb)
C. annually in illness prevention (noun) and gain as much as $200 billion through worker performance improved by simply providing
....companies could save in illness prevention (noun) and gain through performance improved (noun verb)
D. in employee illness prevention and gain as much as $200 billion through improving performance of workers if they simply provided
...companies could save in illness prevention (noun) and gain through improving performance (verb noun)
E. by preventing illness among employees and gain as much as $200 billion through improved worker performance if they simply provided
...companies could save by preventing illness (verb noun) and gain through improved performance (noun)

in the original sentence they does not have a clear antecedent.Also, could save ..by is correct usage rather than in employee ..prevention.

E is clean

got confused with "they" here....
they does refer to companies here. it is clear as employees - object, does not cause a confusion

Using both Each year and annuallly is redundant so A,B,C are out and out of D and E E wins....

My understanding :

Lets make a split : A,D,E n B,C

the stem construction is somewhat like : IF X .................. THEN Y

In the main stem the first half non-underlined portion : .............Companies could save ie in the THEN portion it is ( could save ie could + base form of Verb ) ============= so in the IF portion it should have ( PAST TENSE )

IF (Past tense)....................... THEN ( could + base form of verb)

Thus B,C : ..................providing= eliminated

of A,D,E :

Stem is of the form : Could save by X.................. AND................gain through Y

X & Y = should be Parallel

Preventing Illness : Improved worker perfomance = PARALLELISM MAINTAINED

Leading to E
