(B) it was only the second that an American woman published
(C) the second one only published by an American woman
(D) the second one only that an American woman published
(E) only the second published by an American woman

C implies that only an american writer published the second one.

E implies that the pook that was published was only the second one ever published by an american.

E is correct

A looks like the best choice.

C : the second one only published - here 'only' seems to be misplaced since in 'only the second' only serves to provide emphasis on the fact that it is just the second book. In 'second one only published' only seems to indicate that although the second book was destined for something much better than just being published, it was only published

E: in choice A 'it' is used to refer to book but E does not seem to do that only the second (book) published , maybe it is correct if 'book' is placed as indicated
