I'm looking at some applications and it asks for a few things that aren't required:

- Explain your reason for leaving when listing previous jobs
- Phone number of your previous employers
- What other MBA programs are you applying to
- # of employees at the firm
- And More

Is it okay to leave some of these blank?

Fill them in as completely as possible. It gives the adcom a better picture of you and your work experience, especially if you worked at small companies beforehand.

How about salary from internships? I had an hourly rate. On the app, it only lists salary ranges. Do adcoms prefer conversion of the hourly rate into salary or leave it blank?

Convert it, why would you leave anything blank instead of just tailoring your information to their question?

I agree with this. You have precious little space to convince a member of the committee to accept you into their program. Why would you hurt yourself by withholding information? Most people are trying to find ways to give more information not less.
