I think overall that one area I need to improve on is community activities; which is difficult considering the profession in which I am currently employed. (It is hard to become involved when you are working long hours!!!)

Anyways, thinking about H/S/W, Chicago, Columbia and Northwestern. Am I missing any or should I narrow down at all?

You look like you may have some of the credentials to aim for these 'ultra-elites' but don't take the community involvement piece lightly. The top schools will look for leadership there. You didn't mention post-MBA goals so I can't comment on which schools to add to your list.
