In my opinion, the difference is of one word - Why

As we all know, the theory tested in GMAT is very limited and very basic. All the questions are based on that same theory. The thing about Q50-51 questions is that it is hard to identify the theory they are testing. You might need to use the same simple formula but you will be left wondering which formula to use and how to use it since some manipulation might be required. That is where knowing 'why' helps. You need to understand the 'why' behind everything - every fact, every formula, every process. If you do, then it doesn't matter how they twist the question, you will know what you should do and why.

As always, karishma is right.

you need to know why and you also need to practice a lot.
Going through a lot of different areas and understanding the reason behind 'why' you used what you did is the key.

speed is important as well. If you are able to spend 4-6 minutes on questions because you know you can speed up later and have the belief that you won't mess up, it will give you tremendous confidence.

I believe the improvement only comes by spending enough time on each question, understanding concepts, lingering on problems, not going to the solution till you make enough of a headway into the problem, trying to solve problems in different ways etc. Obviously, all of this takes a lot of time, and you need to weigh whether it is worth the time.

The percentiles drop significantly from Q50 down to Q48. It's usually on the order of 99%ile down to 85%ile and that's really the difference between 2 questions.
It comes down to "why" as karishma mentioned, but also unwavering confidence when answering the question - and no careless mistakes in the interpretation of the question. One area that may be tripped up is DS - if you don't follow your logic cleanly you might get stuck answering the expression they give you and not whether there is sufficient information or not. Or, things get so complicated that you lose track of what satisfies the constraints. Plugging in numbers in an organized format can help you clearly understand a systematic way to go about solving the question.
