(A) which a commoner or foreigner could not enter without any permission, >> 'on pain of death' doesn't seem to be related to this meaning
(B) which a commoner or foreigner could enter without any permission only
(C) which no commoner or foreigner could enter without permission, >> same as A
(D) which, without permission, neither commoner or foreigner could only enter,
(E) which, to enter without permission, neither commoner or foreigner could do,

I first chose C:
"On pain of death" means "if you do X and get caught, you will be judged to death penalty".

A) "could not" and "without" form a double negation (unnecessary)
B) changes the meaning --> it is not that "only IN CASE OF DEATH PENALTY" that someone is allowed to enter. On the contrary: if one person enters the forbidden city, that person can be judged to death penalty.

D) wrongly uses the structure "neither X nor Y"; "could only enter," is awkward

E) wrongly uses the idiom; "to enter without permission" does not 'link' with "neither X nor Y". This sentence is wordy.
