Between C, D and E :

(C) originated from diverse ethnic and cultural heritages and perpetuated
"Originated" is simple-past verb while perpetuated is an adjective. parallelism issue

(D) originating from diverse ethnic and cultural heritages and perpetuated
Both originating as well as perpetuated are adjectives. CORRECT


(E) originating from diverse ethnic and cultural heritages and perpetuating by
"Perpetuating by" is not correct (is awkward) usage)
