My answer would be D.

Reasoning as follows:
A) 'which' is used to start a non-restrictive clause - that would be preferred here
B) use of 'that' is fine, however 'and perpetuated' is left hanging + incorrect use of past tense - these language variations still originate
C) incorrect use of past tense 'originated'
D) correct answer 'originating' present participle + correct use of past tense for 'perpetuated'
E) 'perpetuating' - incorrect present participle + awkward 'perpetuating by geographic isolation'

Between -ed and -ing in this instance I would go for -ed.

I have two reasons, firstly that -ing implies continuous tense, ie. it is still happening, whereas -ed in this situation is simple past tense meaning it happened in the past.
From the meaning of the sentence, as regional dialects are in decline, they can't still be perpetuating, as it is a thing of the past, they would only have perpetuated.

Second and easier to see is parallelism, originated and perpetuated must be parallel. This cuts out A, D and E. Between the remaining two I would choose B for the "that".

"that originated......and [maybe missing a were] perpetuated by geographical isolation" seemed awkward to me.

originating and perpetuated can be parallel....?

I would like to know what the correct answer to this question was.
