great question! took me longer to figure out and parse the sentence correctly 
between B, C, and D
should be " integrated companies, [such as Bethlehem Steel], that once"
the only choice that is most clear is C "with large, integrated.."

in C there is no confusion as to what is being infered in the sentence...
industry changed..with LARG corporation doing blah blah..

"that" is necessary, so it's between B, C & D.
The difference between these three is "as", "with" and "while".
While suggests contrast. There's no contrast - the case of Bethlehem Steel is consistent with the general comment.
With can't be right (with companies have downsized)
Answer must be B.

I picked C. However, can someone explain to me why "with" is wrong in this case? I need to understand this....

I have seen answer choice "with" is corect in some very rare cases but in most of the cases it is wrong. just remember it.

I think it's B because you would say "as large companies have downsized" as opposed to "with large companies have downsized".
If it were "[...] large companies downsizing" then I would agree with C.

Break the sentence
The steel industry "has changed" _______________ once conducted blah blah "have" greatly downsized or shut down
Now fill the blank -
A - It's correct for most part but we will need a "that" before "once"
B - No errors
C - can you pair "with companies" with "have downsized/shut"? It should be something like with companies "shutting down"
D - "while" changes the meaning, has to be "as"
E - Same problem as that of A, it misses "that"

Otherwise it is a run-on sentence

I was able to get down to B, C, and D...but I couldn't figure out how to eliminate between "as", "with", and "While"....
Now I know from the discussion above
"while" - means contrast
"as" - conjunction
"with" - preposition

Excellent. With is used to create prepositional phrases. With is also very popularly used to create subordinate (or dependent clauses) - also known as adverbial clauses.
It would have been ok if the sentence said --
The steel industry has changed radically [..] with large, integrated companies, such as Bethlehem Steel, that once conducted operations from mining at one end of the process to shipping at the other DOWNSIZING or in some cases SHUTTING down altogether
The whole part appearing after WITH has now gotten converted into an adverbial clause modifying the adverb RADICALLY.
I hope you all see the point...
