Spoiler Alert!

I have a question about comparison.

D. would have to know a great deal more than they do now about the social and economic.
Why do we need 'they do' here? OE explains that 'now' can be falsely compared to the knowledge, but do we need that clarification?
If we do wouldn't two examples below be wrong too?

Below two passages are considered correct.
Obesity is more common among American teenagers now than ever before. (o)
We have more engines that are efficient than before. (o)

If above two examples with before are considered correct, wouldn't E be correct to?
E. would have to know a great deal more than now about the social and economic

Are now and before considered different? Both now and before represent time to me.

Let's say now and before are indeed different, then would below be correct?
E. would have to know a great deal more than before about the social and economic

Thanks in advance.

it should be D - you need a "would" in this sentence. The development is a future event, an unreal situation... you need a "would" to account for that

also you need "than they do now"; if you just leave it at "than now" then its comparing "now" with "know" (quite literally) which can't be right

To develop more accurate population forecasts, demographers have to know a great deal more than now about the social and economic determinants of fertility.

A. have to know a great deal more than now about the social and economic - compares knowing with present time , "now"
B. have to know a great deal more than they do now about the social and economical - economical is inappropriate
C. would have to know a great deal more than they do now about the social and economical- economical is inappropriate
D. would have to know a great deal more than they do now about the social and economic - correct option
E. would have to know a great deal more than now about the social and economic compares knowing with present time , "now"

Obesity is more common among American teenagers now than ever before. - compares obesity now & obesity ever before. What are the other options? If "Obesity is more common among American teenagers now than it was ever before" then it will be the best option.
For second sentence also if "We have more engines that are efficient than we had before." - was among the choices then that should be the correct option.
We need to work with only the options given to us.

The Trap or trick to confuse test taker is that, choice B and C tried to match "Social" with "economical" which is wrong beause adding this "al" does not make it right. Social and economic are the right words to use as a combination.
