Should be (D).

Removing stains on the tooth enamel leads to improving tooth appearance. Improving tooth appearance is not a separate result. Therefore 'to' is more appropriate (and not 'and'). That rules out A and B. In C, 'keeping costs down of customers' is awkward and grammatically incorrect.

E is awkwardly worded, changes the meaning by changing the placement of the word 'enough' and is also not parallel in all of its aspects.

D it is.
