Should be (D).

Removing stains on the tooth enamel leads to improving tooth appearance. Improving tooth appearance is not a separate result. Therefore 'to' is more appropriate (and not 'and'). That rules out A and B. In C, 'keeping costs down of customers' is awkward and grammatically incorrect.

E is awkwardly worded, changes the meaning by changing the placement of the word 'enough' and is also not parallel in all of its aspects.

D it is.

This appears to be a GMAT Hacks question of the day. The question appeared on 1/31/2013. The Official Explanation is as follows:

(B) and improve tooth appearance enough for microabrasion techniques to be used less and to keep costs down for---- Same as A. Also to be used less is awkward usage.
(C) for improved tooth appearance, enough for lessening use of microabrasion techniques and for keeping costs down of---- Costs down of customers is incorrect. For improved also is incorrect shift of tenses.
(D) to improve tooth appearance, enough to limit use of microabrasion techniques and to keep costs down for--- CORRECT
(E) to improve enough tooth appearance to use microabrasion techniques less and keep costs down for --- Enough is used incorrectly. enough tooth appearance does not make any sense.
