C and D are close but choose D for the use of "personally" after the negative verb "have not" rather in between "have" and "not".

A/B and E have tense conflicts.

D looks better for me..

You got it correct GMAT TIGER...
OA:
[Reveal] Spoiler:
D

two issues with original sentence. 1) "have seen"..."had witnessed" 2) awkward separation of "have" and "not" in "have personally not witnessed"
D fixes these...

All the other options are wrong due to one of the two reasons listed...
"have seen"/"had witnessed" - A and B gone
present perfect tense "have" with future present "will have" - E gone

I'd chosen C for this answer :)

I chose C for parrallelism. The way the sentenence conveys the meaning, there is no reason to assume that one event occured before the other. So, past participle is incorrect in this case period. The only question is the negative structure "Have not" is always inseparable for it to be gramatically correct? Any help on this.

Even I would have chosen C .. Don't ask me why, just felt that it sounded right .. Anyone there to explain the difference between C and D?

D for me.

We need 'have' because the action of not seeing any evidence and not personally witnessing has been going on in his forty years of experience. Usage of 'had' will be incorrect here because 'had' means the action has already happened before some other action (which is not the case here).

Also, adverb --> modifier to a verb, an adverb, a clause, a phrase, a sentence, an adjective.

'personally' should be a modifier to 'witnessed'

I have seen very little evidence of the expression of the authentic self in American corporations; I won't deny its existence but I had personally not witnessed it once in my forty years of experience.

(A) I have seen very little evidence of the expression of the authentic self in American corporations; I won't deny its existence, but I had personally not witnessed it once in my forty years of experience.

(B) I had seen very little evidence of the expression of the authentic self in American corporations; I won't deny its existence but I have not personally witnessed it once in my forty years of experience.

(C) I have seen very little evidence of the expression of the authentic self in American corporations; I won't deny its existence but I have personally not witnessed it once in my forty years of experience.

(D) I have seen very little evidence of the expression of the authentic self in American corporations; I won't deny its existence but I have not personally witnessed it once in my forty years of experience. - CORRECT

(E) I have seen very little evidence of the expression of the authentic self in American corporations; I won't deny its existence but I will have not personally witnessed it once in my forty years of experience.

C and D are so close... C sounded right to me... But seems D is it...

Is it a rule or sthg that "have" and "not" should not have anything in between them?

I was stuck between C and D.
Since meaning is not changed by moving the modifier 'personally', I agree that (D) is better option.

I also choose D, but my reason is that the adverb personally is nearest to the verb it should modify. So, only choice D is appropriate in term of tense, structure, and meanings.
