choices c and d are same. also "the" is missing so none of the choices seem grammatically correct. best choice would be c/d since the verb "is" is inserted. the verb is missing from the original sentence.

I don't agree on the fact that which is wrongly used here.
which is descriptive and non-determinative in this clause and separated from the main sentence with the appropriate comma's. Correct use of which
I tend to go for A but I'm uncertain on the slammer. If it was used as a "nickname" for the gun I would think it would need capitals like in "The Slammer", not?

Thanks for explaining it. I agree to you.
But another reason why I would have eliminated A would be the missing verb "is"
BTW - "the slammer" is used in all the options so really should not be an issue in this Q
