What's the standard on asking for recommendation letters?

I will be needing to ask for a few recommendation letters for mba apps but what's the standard - should I write it for them and then they sign and send - or should i expect them to write it?

You most definitely want them to write it. There are a number of issues that arise if you try to write it yourself. You should, however create an "application game plan" for yourself and provide that to your recommenders to "help them, help you". That's what our book is all about.
