Well, it would help if you told us what you want to do. Some schools offer more of a general management education while others offer a deeper focus in specific areas.

The 720 gmat score doesnt preclude you from applying anywhere, nor does the GPA. Not sure how the Ph.D plays out - on the one hand, you are certainly well educated, on the other hand, the adcoms may wonder why someone with a Ph.D is pursuing an MBA, I'd be sure to have a good explanation as to why.

The answer to this question is - you should apply where you want to go - not according to statistics etc, go where you believe yuo will fit in and get the most out of.
