Can someone please explain when to use "had" in a gmat SC question? How do you determine whether to use "had" or not?

Had is past perfect. It is used when an action had started and completed before a past action.

e.g. I had dinner before my friends arrived. - In this sentence 'my friends arrived' is the past action. 'I had dinner' is the action which had started and completed before that..
