It's OK to submit a generic letter as long as it answers all of the school's specific questions. That is, you could have them a prepare a *composite* letter that addresses all of the reco questions of your first-choice schools. This will result in a longer letter that contains more material than each school is asking for, but that's preferable to a letter that does not answer all of the questions each school asks. Unfortunately, to optimize your odds at the top schools you really need to give them a letter that answers all their questions (for your safety schools you may not need to).
