A number requires a plural verb 'have'. Options A and B can be eliminated.

Between C, D and E:
C. have encountered a series of unexplained symptoms that have been attributed as - Incorrect - incorrectly suggests that symptoms are attributed as a syndrome. Also, 'attribute to' is the correct idiom.

D. have encountered a series of unexplainable symptoms that have been attributed to - Correct - attribute to is the correct idiom.

E. have encountered unexplainably a series of symptoms that were attributed as - Incorrect - Meaning + Idiom error

IMHO (D)

1. A number of---> Plural ( Left with options C, D and E )
2. Correct idiomatic usage of attribute is - Attribute X to Y ( Left with options A and D )

None but (D) correctly addressed both the issues presented above, so IMHO (D)
