My pick is E. They or their seem to refer to the city's election board which is singular.

Also E is parallel for failing to replace, for failing to investigate.

I will go with E- First of all, failing to.... Second for failing to.... is parallel. None of the others have this parallelism.

D -- Subject-verb issue

E is the best.

IMO E. We need parallelism: "failing to replace"...."failing to investigate"
