I picked D...u can't use "which" in this case because it doesn't make sense and option E is awkward.

Can someone explain the error in option B is it incorrect usage of "which"?

Its between B and D based on parallelism.

Yes, correct

A study by the Ocean Wildlife Campaign urged states to undertake a number of remedies to reverse a decline in the shark population, which includes the establishment of size limits for shark catches, closing state waters for shark fishing during pupping season, and requiring commercial fishers to have federal shark permits.

B. which(shark population) includes establishing limits to the size of sharks that can be caught, closing
logically not correct.
D. including establishing size limits for shark catches, closing
"including" is the only exception to the rule COMMA + ING, so its usage is fine.

A, B and C are have modifier problem. Which seems to be referring to the "decline". The intention is to list out the remedies.

A and B are wrong because of wrong usage of 'which' modifier and SV agreement ( a number of remedies + includes ) plural subject and singular verb.

C and E are wrong as Complex gerund can't be parallel to simple gerund ( after and non underlined part).

Option D is correct with all elements in list are parallel being simple gerund.
