According to Federal Veteran's Administration (VA), a certain county has a relatively high proportion of veterans among its residents, and over the past decade, these veterans have reported to a number of health concerns that have not been adequately addressed. The VA does not have its own hospital in this county.
Plan: For the past 5 years, the VA has earmarked several million dollars to pay all 18 hospitals in this country to fund short-term and long-term care for veterans.
Result: Over the past 5 years, VA surveys have indicated no significant decrease in the number of veterans who say they have health concerns that have not been adequately addressed.
Further information: VA surveys indicate that all veterans are aware of the VA funds that would cover their expenses at county hospitals. Veterans in this county span a variety of ages and a variety of health conditions.

In light of the further information, which of the following, if true, does most to explain the result that followed the implementation of the plan?

A) Some of the veterans in this county are already on Medicare or Medicaid, which can cover a substantial part of hospital care.

explain the result that followed the implementation of the plan?

A) Some of the veterans in this county are already on Medicare or Medicaid, which can cover a substantial part of hospital care.(This covers only some and also does not affect in any way.)
B) Veterans often have special health needs that are not well handled in conventional settings.
C) The percentage of veterans in this county who complain of health concerns that have not been adequately addressed is higher than the corresponding percent in any other county in the nation.(Out of scope)

I have said it before, but I would say again: I love Magoosh! Especially for the well structured and well thought questions.

The question here is: why the number of health concerns that have not been adequately addressed did not decrease?
what if the veterans have long-lasting health issues that cannot be treated?
what if the problems of the veterans cannot be solved at the funded hospitals?

since this is a paradox question, the correct answer should present new information so that when included in the formula below, the paradox would be explained:

Because (correct answer choice), the number of concerns that have not been adequately addressed did not decreased, although the VA funded 18 hospitals to help veterans.

the only option that is in the scope, presents new information, and explains the paradox is B.

Using POE, only B is left even though I have no idea what conventional settings are, and how the settings relate to the veterans and their treatments.

This county has a lot of veterans with health problems. The VA gave millions of dollars to the hospitals to pay for veterans, but this plan didn't work. We have a paradox. What would explain why the veterans' health concerns are not addressed, even though they get their way paid at the hospitals?

(B) is the credited answer. If veterans have special health needs that conventional settings like hospitals don't handle properly, this would explain why the veteran's health concerns have not been addressed.

(A) and (E), rather than resolving the paradox, intensify it. If the veterans have plenty of money for health care (what (A) says) and if the hospitals are successful in treating other people (what (E) says), we would have every reason to think they adequately would address veterans' health concerns. This does not explain why that is not the case.

(C) further describes the intensity of the problem --- the health concerns of veterans in this county have not been properly addressed --- but does not explain why the plan didn't work.

(D) is irrelevant: how money was shuffled around at the federal level is irrelevant to why the hospitals in this county were not able to help these veterans.
