Correct idiomatic usage that conveys clear meaning:- "Not only X----but also Y"
Here both X and Y carry same part of speech. Hence X and Y are parallel.

Not only... but also... is the right idiom structure. D and E don't have that and can be eliminated.

'First one only' in option C is not right.

'only the first' in option B seems to distort the meaning.

So A should be the answer.

imo option A,

not only but also idiom made d and e out, out of remaining 3

A. but also had the distinctive stint of serving as the last Viceroy of India and the first Governor-General of Independent India
B. but also had the distinctive stint of serving as the last Viceroy of India and only the first Governor-General of Independent India ( word order only gives wrong meaning, infact as a matter of fact one more governor general named Rajagopalachari served as governor general following Mountbatten, hahahah)
C. but also had the distinctive stint of serving as the last Viceroy of India and the first one only Governor-General of Independent India ( first one only is literally ugly)

so option a is right answer

This also follows the correct parallel structure but the use of 'only the first' makes changes in the intended meaning, this may suggest that he was the one and only governor general but the meaning is not clearly conveyed in this intent because only the first means that he could have been second gov-gen also but he did not and for these reasons in my opinion this choice is wrong.
C. but also had the distinctive stint of serving as the last Viceroy of India and the first one only Governor-General of Independent India The use of one only here also distorts the meaning and does not convey intended meaning.

Going through the options, D and E are out because not only....but also is an idiom which they dont follow.

Parallel construction i like. lets keep it
