D is best here with the most correct parallelism:

1. be blamed for causing A and B as well as doing C
2. A and B are parallel, too.

D)...

A) is not parallel "...causing hundreds of deaths" ok "...causing injury to thousands" ok "...causing seriously damaging..." not ok !

D, parallelism:

"injury" as a singular noun in A is incorrect ...it should be " injuries to thousands ( thousands of what?!!)"
D has the best parallelism.
