Only D maintains parallelism.
A---->should have been ' serious damage to '.
B---->'causing ' missing before serious.
C----> ' who ' is ambigous.
E----> awkward construction.

Well I think 'mahesh004' meant "doing".

D is the correct one as it's the only choice that maintains llism for .....hundreds of deaths, thousands of injuries.....
