This sentence tests parallelism issues. The ATVs are blamed for causing numerous things. Each of these things needs to be parallel, and the things are not parallel in A, the original.

In D, "deaths" and "injuries" is parallel, and then the structure shifts by using "as well as". When you have "as well as" the object after that phrase does not have to be parallel to the objects in front of that phrase.

D. deaths and thousands of injuries, as well as doing serious damage to
