There is no modifier issue involved in C; It is a complex sentences with two factors connected by a subordinate conjunction 'Although". So the word 'it' needn't be close to the noun it antecedes. .Only thing is that, the generator is not an antecedent but a postcedent, a permissible feature in GMAT

There are two things here
1. generator damaged severely in the hurricane
2. captain repaired it.
The contrast meaning is indicated by the presence of although.

Although severely damaged in the hurricane, the captain was able to repair the generator so that it could power
the hospital until the city’s electricity was restored.

In the given sentence above Although severely damaged ......phrase refers or modifies the captain thereby producing meaning error.
because the captain is not damaged but the generator did.

B: Although severely damaged in the hurricane, the captain had repaired
same error as in A repeats here.
past perfect is misplaced here and should be used with generator damage as captain repaired it later.

C: Although it had been severely damaged in the hurricane, the captain was able to repair
This is the correct choice as this solves the modifier using it to refer generator and gives correct meaning.

D: Severely damaged though it had been in the hurricane, the captain had been able to repair
past perfect tense is wrongly added in later event instead of earlier event.
severely damaged............modifier is not proper and does not convey intended meaning.

E: Damaged severely in the hurricane, the captain was able to repair
same modifier meaning error as in A
past perfect tense is removed altogether adding the confusion in the sequence of events.

adding some more to your doubt of
when u add it in the Although.........phrase it no longer modifies the captain subject of main sentence but only adds the needed info with contrast in meaning i.e., it explains under what circumstance that is even after it is damages still captain repaired it.

If you observe the sentence it is used to refer the generator in non underlined portion of the sentence.
pronoun need not be placed near to the antecedent and still convey the meaning.

one more tip. In any case you feel like, this is not the right place for pronoun then replace the pronoun with antecedent and re-read the sentence. That should make sense if it is correct.

In this case,
Apply the same with option C and observe the whole sentence.

Although generator had been severely damaged in the hurricane, the captain was able to repair the generator so that generator could power the hospital until the city’s electricity was restored.

Doesn't this sound correct. So this is the right option.

Because he lacked working experience, Tom couldn’t get admitted into ISB----Here the noun is postcedent, since Tom appears after the pronoun
Both are okay in GMAT as far as I know.

C: Although it had been severely damaged in the hurricane, the captain was able to repair
- CORRECT

D: Severely damaged though it had been in the hurricane, the captain had been able to repair
- the captain had been is incorrect tense here

E: Damaged severely in the hurricane, the captain was able to repair
- modifier error - the captain is not severely damaged, it is the generator - Hence, A is Out


Hence, Answer is C
