Assuming top schools in the US:
- Since you only mentioned work experience, I'm guessing that you have no relevant teaching or research experience. For PhD admissions, work experience usually carry little weight, and can sometimes work against you.
- 3.5 GPA is not bad, but certainly there are many applicants with much better grades.
- 680 GMAT is not bad, but again there are many applicants with better scores.
- It's not clear if you can get strong letters of recommendation, from professors with good reputation in academic research.
So, from what you wrote, I don't see anything particularly strong, that would make you stand out among all applications top schools get.
Of course there are lots of things that I don't know (e.g., your research interests, if you found schools and faculty with good fit, what is a top or mid level school to you). But from the information you provided, I'd say you can apply to top schools that are an extremely good fit for you, but don't get your hopes up. Even for mid level schools you will have a hard time.
In your place, I would try to get a very high GMAT score before applying. Because, if you can do that, at least there will be a clear strength in your application. And that can make schools take a second look at you.
