B) who might once have died in childhood

C) that as children might once have died

D) who in childhood might have at one time died

E) who, when they were children, might at one time have died.

I think this is easy and i got this correct. I found the OE useful hence posting here for the benefit of others.

I recall seeing this question once, but can't think of the source and the first time I think I picked B, but after reading explanations I remember what is wrong with B now!

Okay, A and C are out because people are "who" not "that"

B "who might ONCE have died"... could they die twice? illogical...

D and E say sort of the same thing but E is wordy...

It should be B.

Reasons:

- "Died of..." is correct idiom.
- "once" is right. "at one time" is wrong in this context.
- "who" is used to modify "People". "that" is not correct.
- Brevity

OA is B.

OE says:

* who should be used with people not that.
* once should be used instead of at one time.
* in childhood (instead of as children or were children) is parallel with into old age
* once is parallel with now

and thing which i found interesting and useful is this:

Parallel structure involves not only how parallel elements are formed but also where they are placed in the sentence:
here adverbs (once and now) should be placed first, followed by verbs (have died and live) and then by prepositional phrases (in childhood and into old age)
