I think C is the right answer.

be is the right subjuctive form for required that.

Between A and C I would choose C

bankers require that.. and (that) a prescribed format is followed.

Between A and C, I would go with A.

C appears to be pasive.

stuck btw A and C...

while C sounds appealing lets look at it further...

what do bankers require....? that information be complete and that it follows a presrcibed format...

well we have "by" mortgage applicants so, the sentence should be in passive form, so C is best...

I almost picked A, but then if I recall, "by" signals a passive form...so...we should pick the passive mood...

Tough one!

Require that needs "be" + verbs

C doesn't sound parallel with "be" + "a"....

However, A is parallel with two verbs...."financial information be complete and follow a prescribed format"

A)..."require that" requires the subjunctive mood. both "be" and "follow" are in the subjunctive mood. C) uses passive as well as active voice.
