Can someone clarify why it’s “D” and not “E”?

(E) Mt. Vesuvius has erupted cataclysmically many times since its most famous one in 79 AD

Use of one here is wrong.

a depression measuring 2,100 feet across and 750 feet deep - this entire noun phrase is an appositive and hence can be ignored. This phrases simply describes crate.
Opening Verb-ed modified modifies the noun or noun phrase right after the modifier. Hence, this modified must modify a volcano. Only Option D & E eliminates this modifier error.

(A) many cataclysmic explosions have been associated with Mt. Vesuvius since the volcano’s most famous eruption in 79 A.D.
1. Modifier Error as stated abolve.

(B) they have associated many cataclysmic explosions with Mt. Vesuvius since its most famous eruption in 79 A.D.
1. Modifier Error as stated abolve.
2. Pronoun Error: Pronoun they do no have an antecedent.

(C) many cataclysmic explosions were associated with Mt. Vesuvius since its most famous eruption in 79 A.D.
1. Modifier Error as stated abolve.
2. Tense Error: Usage of past tense were is incorrect with since; instead we must use present perfect.

(D) Mt. Vesuvius has exploded cataclysmically many times since its most famous eruption in 79 A.D.
The opening modifier correctly modifies Mt. Vesuvius. The usage of present perfect tense is correct in this case.

(E) Mt. Vesuvius has erupted cataclysmically many times since its most famous one in 79 AD
The Modifier & Tense errors do not exist in this option.
Pronoun Error: 'One' does not have a antecedent. It might appear that one denotes eruption but the word eruption is not present in the sentence; instead the verb erupted is present but as per Grammar rules a pronoun can only refer to a noun, noun phrase, or pronoun and not a verb.
