Beside has been added i do not see any errors
Why the correct answer reverse the sentence order ?

You are right, has is the error in main sentence. But I must say that the order in C is actually the correct order and that in A is reversed. The order in C is straight and crisp.
Which one would you prefer the waiter saying to you?

1. Here's your pizza, to which mushroom , jalapenos and cheese are added.

2. Here's your pizza, to which are added mushroom , jalapenos and cheese.

Yes C is the correct the answer because it corrects "has been added" to plural "have been added"
