+1 for A.

A. surrounding internal organs, as efficiently as men do --> Proper comparison between two entity, and intact meaning

Hence, A.

Due to differences in physiology and metabolic response, women who diet and exercise do not lose visceral fat, the adipose tissue surrounding internal organs, as efficiently as men do.

A. surrounding internal organs, as efficiently as men do --> correct

B. surrounding internal organs, as efficiently as men lose weight --> meaning change "lose fat vs weight"

C. that surrounds internal organs, as efficient as men --> doesn't make sense: women do not lose fat as efficient as men

D. surrounding internal organs, as men's efficiency at visceral fat loss

E. that surround internal organs, as efficiently as men do --> singular subject "tissue"
