A. was - Blood type was O -ve so now did it change?
B. had been - Past Perfect can be used when something happened before something in the past
C. has been - Present perfect is used for something that started in the past and touches the present
D. is - Simple present (something that is generally or always true)
E. was being - unidiomatic

Blood type will always be the same. Hence the tense is shows this clearly.

Option D
