Even the most advanced neuro-diagnostic tool will sometimes fail to identify a lesion when it is present or suggest that there is one when one is not.

A. a lesion when it is present or suggest that there is one - it can refer to tool or lesion ;there ?

B. when a lesion is present or suggest that there is one -identify when? when represents a moment; there : no place is mentioned

C. a lesion when one is present or suggest that one is present - wordy but no ambiguity so far-keep it

D. when a lesion is present or suggest its presence- same as B for when ; suggest to /that ; its refers to?

E. the presence of a lesion when it is there or suggest its presence - identify the presence or suggest its presence- sounds parallel but still plenty wrong- reject

Hence C is right answer

IMO-C

A. a lesion when it is present or suggest that there is one
Incorrect- two different pronouns it and one refer to noun a lesion

B. when a lesion is present or suggest that there is one
Incorrect -use of when before a lesion is changing meaning of the sentence

C. a lesion when one is present or suggest that one is present
Correct one refer to a lesion and placement of when is correct

D. when a lesion is present or suggest its presence
Incorrect placement of when is incorrect

E. the presence of a lesion when it is there or suggest its presence
Incorrect- it is changing intended meaning of the sentence
