I guess the answer is based on the parallelism between implementing and demanding, thus it should be, "B".

wow it really is a tricky one, spent full 2 mins on finding fault.
eliminating modifiers helped to reduce the clutter.
as...IMF implementing X and demanding Y.

Also,

among
(i) improved economic outcomes, (ii) improving economic outcomes and (iii) economically improved outcomes:

In (i) ' economic outcomes' is the main noun (which are improved) -> correct
(ii) improving is the main noun (or gerund) (economic outcomes is the object) -> incorrect
(iii) 'economically' changes meaning altogether -> incorrect

This is a tricky parallelism questions: The IMF has been implementing <blah..> and demand

so demanding is in choice B. the tricky part of the issue is we tend to focus on improve/improving.

C. improving economic outcomes and a demand for .........>wrong parallel element ,false parallelism(as a way......and.....a demand)

D. economically improved outcomes and demands ......>parallel error,Wrong for the same reason as Answer Choice A

E. improved economic outcomes and demanded .........>false parallelism(You always must figure out which grammatical structures are logically parallel before making them structurally parallel.)

Intended meaning:International Monetary Fund do two tasks.....Implement....... and.....demand (so,parallel element is verb)

Parallelism:The presence of "and"(Parallel Marker) in the original sentence indicates that parallelism issue tested here,so resolving parallel issue(Is it correct parallelism?/Is it false parallelism?/Is the parallelism really needed here?) is the first task to do.

Only answer Choice B matched with correct parallelism:

For more than five decades, the International Monetary Fund has been implementing its structural adjustment policies in developing countries as a way toward improved economic outcomes and demanding financial transparency from participating governments.
