I was able to get this for the SAT's, even though I probibaly shouldn't have. What is the method in order to apply for this?

Go to the official site, http://www.mba.com. They'll have the info there.

I would imagine that if you have a disability that warrants extra time, you'd need to provide proof and make a written request.

Getting approved for the extra time is much more difficult than SAT. I was approved for SAT bc of ADD, but I submitted all my documentation, and they partially denied me and asked for more.

However, with all the time that has gone by I can't waste any longer. So I'm taking it with normal time in 2 weeks, currently scoring around 600. I'm just studing hard and crossing my fingers at this point.
